Healthcare Common Procedure Coding System (HCPCS) Level II Code

HCPCS codes are 5-digit alphanumeric codes that are assigned to drugs by the Centers for Medicare and Medicaid Services (CMS). ALOXI® has been assigned the following unique HCPCS codes in the “J” series (known as J codes):

J2469 Injection, palonosetron HCl, 25 mcg

When billing for a drug, payors require physicians to indicate on the claim form the quantity of product administered to the patient expressed in the number of units described by the HCPCS code. Because the HCPCS code for ALOXI® is expressed as 25 mcg, the amount of ALOXI® administered to a patient is expressed in multiples of 25 mcg on the claim form.

Indication

ALOXI® (palonosetron HCl) injection 0.25 mg is indicated for the prevention of acute and delayed nausea and vomiting associated with initial and repeat courses of moderately emetogenic chemotherapy, and acute nausea and vomiting associated with initial and repeat courses of highly emetogenic chemotherapy.
